Nightmare Boy is a 2D action-adventure metroidvania set in Noctum, where the player seeks to save the region from a self-proclaimed ruler. The journey brings Billy into contact with unusual friends and foes, while enemies drawn from nightmares stand between him and his goal.

The story unfolds in Donorok, a land in Noctum destabilized after King Aster disappears. Monsters, Mongos and Dreamers are in conflict, and Donorok is gradually absorbing terrain from surrounding regions. Balder has seized power and ordered monsters to capture children who fall into the Nightmare; Billy is central to the continuation of his rule.
